List each defect category or cause and how many times it occurred over one period. The table ranks them and the chart sorts the bars high-to-low with a cumulative line. Where that line crosses 80% separates the vital few (worth fixing first) from the trivial many. Feed the top categories into a 5 Whys or Fishbone to find their root cause.
