How to use
Fill the IS and IS NOT columns to draw a tight boundary around the problem. Then ask what is different or changed about the IS cases versus the IS NOT cases — those distinctions point straight at the likely cause. Feed the result into a 5 Whys, Fishbone, or 8D.
